This scheme was set up specifically to provide support services to older people from the African / Caribbean and other minority ethnic communities while living in their own self contained accommodation (e.g. local authority housing, housing association, private rented and owner occupied).
Aims and Objectives
- To provide support and assistance to older people from African / Caribbean and other minority ethnic communities who are isolated
- To help older people gain access to basic services in order to improve the quality of their lives while living in their own self-contained accommodation (e.g. local authority, housing association, private rented and owner occupied)
- To help address specific issues of this client group including cultural, religious, and language needs through casework
- To network with groups / individuals who can help clients combat isolation and situations they may find difficult to tackle on their own
- To empower clients to develop skills to manage their lives in their home / community to the extent that the support can, where possible eventually be withdrawn
What does the scheme provide
The scheme assists you to gain access to the following:
- Meals on wheels
- Transport to and from your home
- Advice on services within the community
- Assistance to meet your cultural needs i.e. meals, social activities etc
- Introduce you to services in the community
- Liaise with external agencies on your behalf
- Arrange catering and other services
- Introduce you to new services and activities
- Help you to overcome social isolation or deprivation where necessary

- People from African / Caribbean and other ethnic minority communities
- People aged 50 and over
- Carers of African / Caribbean and ethnic minority communities person
- People living in self contained accommodation (Local Authority, Housing Association, Private, Rented or owner occupied)
- People living in London Borough of Ealing in need of support

What happens after you have been referred to us?
A staff member will contact you to arrange an appointment for the following:
- Home visit to assess your needs
- Agree a support plan with you / referral agency / carers
- Action Plan Support as agreed
- Monitor / review support plan at regular intervals
- Decision on next course of action
